Are you eligible?

The Transport Act 2000 sets out seven qualifying conditions for disability passes and your disability must be long term or expected to last at least 12 months.

The seven categories are:

A. Registered blind or partially sighted

B. Severely or profoundly deaf

C. Without speech

D. Disability or injury which has a substantial and adverse effect on ability to walk

E. Loss of the use of both arms or without arms

F. Learning disability

G. Would be refused a driving licence on the grounds of medical fitness, for example:

  • Uncontrolled epilepsy
  • Severe mental disorder
  • Liable to sudden attacks of giddiness or fainting
  • Established dementia conditions
  • Inability to read a registration plate in good light at 20.5m (with lenses)

Step 3

Post the form with the requested evidence to West Midlands Combined Authority, PO BOX 9421, Birmingham, B19 3TR.

Remember to include a passport size photo, read and sign the declaration, and include all the documents the form asks for to ensure your application is processed.

We aim to process applications within six weeks. We may write to you to request more information or ask you to attend a medical assessment.

Lost your pass?

Don't worry, you can apply for a replacement online. Just use the link below and click 'Apply online' to begin. There will be an administration fee, so make sure you have a credit or debit card to hand. 

Replace your pass

By phone - Please call 0345 303 6760 to apply for a replacement pass (an administration charge will apply).

In person - Visit a Travel Centre and order and pay for your replacement today. Your pass will be posted first class to your address. (Birmingham New Street and Wolverhampton Bus Station only).

By post - Download this form, include a cheque or postal order payable to West Midlands Combined Authority, and your photograph (if you would like to change it) and post to Ticketing Team, 16 Summer Lane, Birmingham, B19 3SD.
