If your pass has been stolen and you have a crime reference number a replacement is free of charge.

It's a bit slower, but you can apply for a pass or replace a lost pass using the downloadable forms below.

Post the form to us or prove yourself at a check and send outlet with:

1. Proof of age - one of the following: passport, medical card, driving licence, birth certificate.
For female applicants who have a different surname to their birth certificate - a marriage certificate.


2. Proof of address - Driving license or one of the following: council tax bill, utility bill, bank statement, NHS or DWP letter - no more than six months old.

The check and send location will stamp your form and may even post it for you. If they don't post it, please return it to: Centro Ticketing Team, 16 Summer Lane, Birmingham, B19 3SD.

If you are posting the application to us please send copies of your documents, not originals.


To apply for a replacement if you have lost your pass download this form, include a cheque or postal order payable to Centro, and post to Centro Ticketing Team, 16 Summer Lane, Birmingham, B19 3SD.
